This is an interesting and quite common problem in leukocytoclastic vasculitis with ulceration. The white reticulated pattern are lesions of atrophy blanche(white scar). They basically represent areas where tissue necrosis has occurred with subsequent healing with atrophy and avascular scars. The ulcerations appear somewhat superficial, but are exquisitely painful and very slow to heal. Notice the presence of varicosities are often a part of the clinical pattern of this disease. The actual pathology is occurring in the post-capillary venules. Leg ulcers are a significant problem leading to major inconvenience, discomfort and inability to work or function. Most leg ulcers occur about the ankles and are secondary to varicosities (varicose veins).
